Moderator name appears more than once, when listing forum moderators.

Notice the moderator name appears 2 times.

That's probably because you have inserted it twice, just delete the copy

I've seen this happne to me various time, my connection would stall and I would click the button again and I would see two chris's or 2 cpet's or 2 mark's but a simple removal of one fixes the issue. This isn't a bug.

A nice fix would have memht check the input to see if there is any doubles and give an error.
