you use memht version 2.9 and you install mod approve users 3.2
no work of course

Upgrade to MemHT Portal 3.2:
http://www.memht.com/lastrelease.html

and then reinstall the Approve users 3.2 (REPLACE THE FILES WITHOUT EXECUTING THE INSTALLATION PROCESS)
